How scratch and dented are the S&D Kindles offered?
Can hardly get noticed or really bad?

It seems that it varies widely. People have reported receiving ones with barely perceptible imperfections to clearly visible scratches in the screen. You might want to review the forums for past comments by S&D buyers if you would like more personal experiences. Buying S&D items obviously always has a fair level of risk.
This is the caveat added to the S&D Kindle listings:
Hey, you there! These items are expected to have a moderate level of wear & tear including (but not limited to) scratches, dents, and dings. Outside of their physical appearance, they have been tested to be in full working condition.
