I have that I use frequently. Really happy with this item. I always use heat. However, I experimented to answer your question. Definitely will massage without heat.

Is this meant to be used as displayed? With the pad laid on your back as you lay on your stomach? A lot of heating pads say to not lay on them for risk of a burn but most massage pads don’t apply the needed pressure by just draping them over an area.

I have one. The weight actually works to make the heat and massage connect with your back as pictured. Highly recommend!
